FeldCare Connects, a pioneering referral network, seeks an exceptional Home Health Occupational Therapist to deliver premier care and passion for adults of all ages.

This position is for an Independent Contractor serving the Palmdale area specifically.

The Home Health Occupational Therapist assesses patient care needs relating to functionality status, activities of daily living, fine motor coordination, home assessments, and adaptive equipment. Based on this assessment, the Occupational Therapist determines a treatment plan, performs interventions, and evaluates patient progress.

Responsibilities include:

  • Providing services defined by state laws governing Occupational Therapy practice, in accordance with the plan of care, and in coordination with other healthcare team members.
  • Leading patients in therapeutic, creative, and self-care activities to improve functioning and safety.
  • Observing, recording, and reporting patient responses to treatment and changes to their condition.
  • Instructing patients, families, caregivers, and the healthcare team in therapy areas where they can participate in assisting the patient.

Qualifications:

  • Ability and enthusiasm in working with all ages.
  • OCCUPATIONAL THERAPIST LICENSE AND REGISTRATION BY THE STATE REQUIRED.
  • Completion of an accredited OCCUPATIONAL THERAPY PROGRAM.
  • Bilingual is a plus.
