Job Summary

We are seeking a compassionate and skilled Registered Nurse to join our team as a Contingent Home Care Nurse. As a member of our team, you will provide one-to-one care to patients in their homes, utilizing cutting-edge technology and clinical knowledge to deliver exceptional patient outcomes.

Responsibilities
  • Provide comprehensive care to patients in their homes, including assessment, care planning, and intervention.
  • Collaborate with healthcare teams to ensure seamless care transitions and optimal patient outcomes.
  • Utilize advanced technology, including our Home Care Connect virtual care program, to enhance patient care and reduce preventable ER visits and hospitalizations.
  • Participate in ongoing education and training to stay current with best practices and industry developments.
Requirements
  • Graduate of an approved nursing education program.
  • Licensure as a Registered Nurse in the State of Georgia.
  • One year of experience as a professional acute care nurse.
  • Home care experience preferred but not required.
