About the Role

We are seeking a dedicated and compassionate Substance Use Disorder Counselor to join our team at Wellpath. As a key member of our interdisciplinary team, you will play a vital role in evaluating, treating, and providing support for individuals on their recovery journey from substance use disorders.

Key Responsibilities:

  • Administer screening and assessment tools to diagnose substance use disorders within the scope of practice.
  • Contribute to treatment plans and participate in multidisciplinary team meetings to evaluate and treat patients with Substance Use Disorder (SUD).
  • Deliver group and individual services for individuals with substance use disorders, ensuring thorough, accurate, and timely documentation while maintaining confidentiality and adhering to HIPAA requirements.
  • Collect and analyze data related to provided services, assisting in report preparation as necessary for contracts, grants, and accreditation.
  • Plan for the continuity of care upon the individual's release from the facility, providing crisis response and intervention services, and fulfilling other assigned duties.

Qualifications & Requirements:

  • High School diploma and certification required.
  • Bachelor's degree in health services field is preferred.
  • Experience providing SUD counseling required.
  • Ability to practice independently preferred.
  • Licensure or certification or license/certification eligible as required by state in which employed.
