Position Title: Nursing Case Manager

Location: New Hampshire

Compensation: $38-$43 per hour

Role Summary: The Nursing Case Manager is tasked with overseeing a specific caseload of patients, delivering direct nursing care and comprehensive health services to individuals within the community.

Key Responsibilities:

  • Manage and coordinate patient care effectively.
  • Provide direct nursing services to patients in their homes.
  • Conduct assessments and develop care plans tailored to individual needs.
  • Collaborate with healthcare professionals to ensure optimal patient outcomes.

Qualifications:

  • Valid nursing license in the state of New Hampshire.
  • Minimum of 2 years of experience in skilled nursing care preferred.
  • At least 1 year of experience in a supervisory nursing role preferred.
  • Recent experience in wound care is essential.
  • Ability to lift up to 50 pounds without restrictions.
  • Current CPR certification is required.
  • Reliable transportation is necessary for travel to patient locations.
