Job Summary

We are seeking a skilled Registered Nurse Case Manager to join our team at Interim HealthCare of New Hampshire.

Job Responsibilities

  • Manage a defined caseload of patients, providing direct patient care and comprehensive nursing services.
  • Collaborate with healthcare professionals to develop and implement individualized care plans.
  • Conduct thorough assessments, identify patient needs, and coordinate care services.
  • Provide education and support to patients and their families to promote health and well-being.
  • Document patient progress, maintain accurate records, and communicate effectively with the healthcare team.

Requirements

  • Valid nursing license in the state of NH.
  • 2+ years of experience in skilled care, preferably with wound care experience.
  • 1+ year of experience in nursing supervision.
  • Current CPR certification and reliable transportation.
