- Conduzir campanhas de divulgação, utilizando estratégias como visitas porta a porta, telefonemas e e-mails para aumentar a conscientização dos residentes sobre os serviços disponíveis.
- Preparar e enviar toda a documentação necessária conforme exigido pelas agências pertinentes.
- Auxiliar em intervenções em situações de crise de complexidade moderada a alta, garantindo o acompanhamento adequado.
- Implementar políticas e procedimentos de relatório obrigatório.
GERENCIAMENTO DE PARCERIAS
- Compreender os objetivos de parceria e critérios de seleção, promovendo ativamente parcerias em nível de propriedade.
- Colaborar com agências comunitárias locais para fortalecer a rede de apoio.
- Monitorar a qualidade dos serviços prestados pelos parceiros nas propriedades designadas.
ATENDIMENTO AO CLIENTE
- Realizar avaliações de necessidades em nível de propriedade e acompanhar as ações resultantes.
- Participar do desenvolvimento de planos para a propriedade, respondendo às necessidades dos residentes, incluindo populações de alta acuidade.
- Comunicar e executar ações com base nas avaliações realizadas.
DECISÃO BASEADA EM DADOS
- Manter registros precisos em nível de propriedade, incluindo a inserção de dados e o rastreamento de atividades de serviço.
- Garantir a entrega de relatórios em conformidade com as políticas e procedimentos estabelecidos.

- Participar de reuniões de equipe e treinamentos, promovendo um ambiente colaborativo.

QUALIFICAÇÕES
Para ter sucesso nesta função, o candidato deve atender aos seguintes requisitos:
- Graduação em Educação, Serviço Social, Sociologia, Gerontologia ou área relacionada, com pelo menos 1 ano de experiência em desenvolvimento comunitário ou serviços sociais.
- Experiência mínima de 1 ano em áreas relacionadas, como apoio a famílias, jovens ou idosos.
- Experiência em coordenação de programas e serviços.
- Colaboração com provedores de serviços locais.
COMPETÊNCIAS NECESSÁRIAS
- Fluência em inglês e espanhol, tanto escrita quanto verbal.
- Excelentes habilidades de atendimento ao cliente e resolução de conflitos.
