Job Title: Part Time Care Center RN

At Hospice of Darke County, we are seeking a compassionate and skilled Part Time Care Center RN to join our team. As a Part Time Care Center RN, you will provide direct patient care in our Inpatient Care Center, working closely with our interdisciplinary team to deliver high-quality care to our patients and their families.

Responsibilities:
  • Provide competent nursing care to assigned patients through skilled symptom management, emotional support, and timely response to patient and family needs.
  • Coordinate IDG planning for assigned patients, participating in the IDG analysis of the patient resulting in the development, implementation, and revision of the IDG Plan of Care.
  • Teach, supervise, and evaluate the care provided by volunteers, hospice aides, and primary caregivers.
  • Facilitate continuity of care by coordinating transfer/admission of home-care patients to in-patient setting and vice versa.
  • Explain hospice services and benefits in regards to Medicare, Medicaid, and Private Insurances, and knows when to use Palliative Care Interventions and ABNs.
  • Obtain and enter orders from Medical Directors and Physicians when deemed appropriate.
  • Order appropriate Durable Medical Equipment (DME) for patients.
  • Perform competent initial nursing assessments.
  • Work evenings and weekends as needed to meet the needs of patients and families and agency backup schedule.
  • Complete all documentation as required by the organization in compliance with Agency policy, accreditation bodies, and governing requirements.
  • Maintain clinical competency for assigned responsibilities and procedures.
Requirements:
  • Graduation from an approved School of Nursing.
  • Current RN licensure in the State of Ohio.
  • Minimum of two years of experience in Medical/Surgical, Oncology, or community health nursing.
  • Hospice experience preferred.
  • Shall have a valid driver's license.
  • Excellent customer service skills.
  • Detail-oriented with excellent communication, documentation, and organizational skills.
  • Ability to follow instructions.
  • Ability to work independently without supervision.
  • Travel to other sites as needed. Travel may be in inclement weather.
  • Ability to lift 25 lbs of supplies or equipment.
  • Requires various amounts of sitting, standing, twisting, turning, bending, lifting, and walking.
  • Ability to assist with patient care activities such as ambulation, transfers, and positioning.
