EverHeart Hospice is seeking a skilled and compassionate Care Center Nurse to join our team. As a Care Center Nurse, you will be responsible for providing high-quality patient care in our Inpatient Care Center.
Key Responsibilities- Direct Patient Care: Provide competent nursing care to assigned patients through skilled symptom management, emotional support, and timely response to patient and family needs.
- Plan of Care: Develop, implement, and revise the Plan of Care for assigned patients in collaboration with the patient's physician and/or Medical Director.
- Coordination of Care: Coordinate the transfer/admission of home-care patients to in-patient setting and vice versa, ensuring continuity of care.
- Education and Training: Teach, supervise, and evaluate the care provided by volunteers, hospice aides, and primary caregivers.
- Communication: Explain hospice services and benefits to patients and families, including Medicare, Medicaid, and private insurance options.
- Documentation: Complete all documentation as required by the organization in compliance with Agency policy, accreditation bodies, and governing requirements.
- Education: Graduation from an approved School of Nursing.
- Experience: Minimum of two years of experience in Medical-Surgical, Oncology, or community health nursing.
- Licensure: Current RN licensure in the State of Ohio and the eligibility to obtain licensure in the State of Indiana or current RN licensure in the State of Indiana and the eligibility to obtain licensure in the State of Ohio.
