Social Worker Program Coordinator

This position is located within the Department of Veterans Affairs (VA), Veterans Health Administration (VHA), VA Mid-Atlantic Health Care Network (VISN 6), Hampton Veterans Affairs Medical Center (HVAMC), under the Mental Health and Behavioral Sciences Service Line (MHBS).
The incumbent is responsible for the management of the administrative and clinical aspects of the Primary Care-Mental Health Integration (PCMHI) Program at the Hampton VA Medical Center.
Responsibilities- Administrative (Supervisory) Responsibilities (50%):
- Standardization of training and programming improves efficiency and effectiveness of programming.
- The incumbent is responsible for ensuring standardization of processes within each key clinical program noted below, including the development of relevant SOPs and job aids.
- The incumbent is responsible for strategic planning to ensure the provision of high-quality services to meet the needs of the Veterans being served in the clinical programs detailed below.
- Clinical Responsibilities (40%):
- Functions as a Licensed Independent Practitioner at the advanced practice level and contributes to the Medical Center Professional Councils.
- Completes bio-psychosocial patient evaluations and periodic reassessments of each patient served, which provides a comprehensive database to identify psychological, social, and vocational needs and the appropriate treatment and services to be provided.
- Provides complex case management services/lead coordination for Veterans with complex needs and evaluates walk-in Veterans providing same-day service.
- Academic/Teaching/Training (10%):
- Provides consultation and training to other staff on the special needs of veterans.
- Provides education to Veterans on VHA health care services available at the facility and about services available within the VISN and in the local community.
Monday-Friday 8:00am-4:30pm
