Job Title: Home Infusion Nurse

We are seeking a skilled and compassionate Home Infusion Nurse to join our team at Naven Health. As a Home Infusion Nurse, you will play a vital role in delivering high-quality infusion care to patients in the comfort of their own homes.

Job Summary:

The Home Infusion Nurse is responsible for coordinating and providing direct infusion care to patients, ensuring their safety and well-being throughout the treatment process. You will work closely with our multidisciplinary team to develop and implement individualized care plans, providing education and support to patients and their families.

Key Responsibilities:
  • Plan, provide, and document professional nursing care for patients in accordance with physician orders and established policies and procedures.
  • Use professional judgment to develop and individualize patient care plans, taking into account their unique needs and responses to treatment.
  • Perform treatments, administer medications, and educate patients and families/caregivers on infusion therapy and care needs.
  • Assess patient needs and physical status through health data assessment and patient interview, re-evaluating as necessary to ensure optimal care.
  • Communicate effectively with patients, caregivers, healthcare team members, physicians, and administrators to ensure continuity of care.
  • Complete clinical documentation in real-time, following nursing standards of practice and adhering to Naven Health policies.
  • Coordinate discharge planning, preparing discharge summaries with patient instructions and transferring care as needed.
Requirements:
  • Graduate of an accredited School of Nursing.
  • Current active and unrestricted registered nurse license.
  • At least 2 years of clinical experience working as a RN in an area requiring training and hands-on experience in infusion therapies.
  • Current certification in Basic Life Support for Health Care Professionals.
  • Valid Drivers License and proof of liability insurance with access to a vehicle for business travel.
Preferred Qualifications:
  • Bachelor of Science in Nursing from an accredited school of nursing.
  • CRNI, IgCN, OCN highly preferred.
