About the Role

We are seeking a skilled Licensed Practical Nurse to join our team at Providence Home Health in Mount Angel, OR. As a LPN, you will work alongside an interdisciplinary team to provide high-quality care to patients in their homes.

Key Responsibilities
  • Provide whole-person care to patients in need, working collaboratively with the healthcare team to develop and implement individualized care plans.
  • Conduct thorough assessments of patients' physical, emotional, and social needs, identifying areas for improvement and developing strategies to address them.
  • Administer medications, perform treatments, and provide education to patients and their families on disease management, self-care, and wellness.
  • Collaborate with other healthcare professionals to ensure seamless transitions of care and optimal patient outcomes.
  • Document patient progress, identifying areas for improvement and developing strategies to address them.
Requirements
  • Current Oregon Licensed Practical Nurse (LPN) licensure.
  • 2 years of patient care experience in a skilled nursing facility or medical clinic.
  • Ability to work independently and as part of a team, with excellent communication and interpersonal skills.
  • Strong problem-solving and critical thinking skills, with the ability to prioritize tasks and manage time effectively.
  • Basic Life Support (BLS) certification, with the ability to obtain National Provider BLS certification within 30 days of hire.
Preferred Qualifications
  • Experience in Acute Care, Home Health, or Public Health nursing.
