Job Summary:

The Licensed Practical Nurse (LPN) will be responsible for providing direct patient care, administering medications, and performing various medical procedures. You will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each resident.

Responsibilities:
  • Provide direct patient care, including vital sign monitoring, wound care, and medication administration.
  • Develop and implement individualized care plans in collaboration with the interdisciplinary team.
  • Administer medications and treatments as prescribed by physicians.
  • Perform various medical procedures, such as injections and blood draws.
  • Document patient information and progress in the medical record.
Requirements:
  • Valid LPN license in the state of Pennsylvania.
  • 1+ year of experience as a LPN in a long-term care setting.
  • Ability to work effectively in a fast-paced environment.
  • Strong communication and interpersonal skills.
  • Ability to lift 25 pounds and stand for long periods.
