Role Overview:

As a member of our Senior Community Care (SCC) team, you will provide essential care to patients in various settings, including home care, nursing homes, and assisted living facilities. This impactful work enhances access to care and supports patients in their health journeys.


Key Responsibilities:
  • Primary Care Delivery:
    • Deliver high-quality, cost-effective care to assigned members.
    • Manage medical and behavioral health conditions in collaboration with physicians or specialists.
    • Conduct thorough assessments and document findings in compliance with regulations.
    • Ensure accurate ICD10 coding for all diagnoses.
    • Perform medication reviews and reconciliations as needed.
    • Facilitate care plans by engaging with facility staff and families.
    • Evaluate and adjust care plans based on effectiveness and member needs.
  • Care Coordination:
    • Understand and communicate payer benefits and policies effectively.
    • Assess the necessity of ancillary services and coordinate care transitions.
    • Monitor member needs and adjust care plans accordingly.
    • Review treatment orders for appropriateness and effectiveness.
  • Program Enhancement:
    • Maintain effective communication with internal and external stakeholders.
    • Promote the SCC program in assigned facilities to foster membership growth.
    • Demonstrate creativity in developing improved methods for care delivery.
    • Work independently with minimal supervision.

Qualifications:

Required:

  • Certified Nurse Practitioner through a national board.
  • Graduate of an accredited MSN program with board certification.
  • Active and unrestricted state license.
  • Current DEA licensure or ability to obtain post-hire.
  • Reliable transportation for travel to patient sites.
  • Ability to lift 30 pounds and navigate various environments.
  • Availability to work flexible hours, including evenings and weekends.

Preferred:

  • 1+ years of experience in Long Term Care.
  • Knowledge of Geriatrics and Chronic Illness.
  • Proficient in electronic medical records.
  • Strong customer relationship skills.

Compensation:

The hourly range for this role is $42.31 to $83.27, based on various factors including local labor markets and experience.
