Job Summary:We are looking for a compassionate and experienced LPN to provide high-quality patient care to our residents. As a Licensed Practical Nurse, you will be responsible for administering medications, monitoring patient health, and collaborating with our interdisciplinary team to ensure the best possible outcomes.
Responsibilities:- Provide skilled nursing care to community residents, including administering medications and monitoring patient health.
- Collaborate with our interdisciplinary team to develop and implement individualized care plans.
- Document all aspects of patient care in accordance with DDD and Community Residence requirements.
- Monitor and coordinate care with Community Resident Assistants.
- Provide staff training to promote professional development and optimize patient care.
- Engage in professional relationships with community health professionals.
- Review residents' physical and medication records.
- Ensure implementation of medically prescribed diets and exercise programs.
- Consult with medical staff on medical, health, and safety issues.
- Respond to crises and intervene as needed.
- High School Diploma or G.E.D required.
- Minimum of one year experience working with persons with developmental disabilities preferred.
- Current Licensed Practical Nursing license issued by New Jersey State Board of Nursing.
