Job SummaryWe are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to provide one-to-one patient care in a home environment. As a Private Duty Nurse, you will assist our patients with transitioning home from the hospital and provide the continuous care needed to remain at home with their loved ones.
Responsibilities- Implement physicians' prescribed plans of care and document care provided in a system of record.
- Administer prescribed medication, treatments, and therapies.
- Conduct patient assessments and coordinate care with healthcare teams.
- Provide health promotion, teaching, and training to family members.
- Active, unencumbered LPN license in the state of practice.
- Current CPR certification from the American Red Cross or American Heart Association.
- TB skin test (current within the last 12 months).
- At least 6 months of hands-on nursing experience.
- Health, dental, vision, and life insurance.
