Responsibilities:- Evaluate the quality of care for long-term care residents
- Develop care plans with short-term and long-term goals
- Conduct meetings with treatment teams, caretakers, and families
- Collaborate with specialists in the community for achieving care plan goals
- Graduate of an accredited nursing program
- Valid, unrestricted RN license in the state
- Previous MDS experience preferred
- Previous experience in Long Term care preferred, but not required
