Responsibilities
  • Assess and evaluate patients to develop personalized care plans
  • Provide physical therapy services, including exercises, stretches, and other interventions to improve mobility and function
  • Communicate with patients, families, and healthcare teams to ensure seamless care coordination
  • Document patient progress and outcomes to inform care decisions
  • Collaborate with other healthcare professionals to ensure comprehensive care
Requirements
  • Current, unencumbered State Physical Therapy License
  • One year experience as a home care professional Physical Therapist
  • Ability to assess and document patient needs and formulate individualized patient care plans
  • Proficiency in clinical skills and Microsoft Office
  • Ability to drive in a car 2-4 hours per day
