LHC Group is seeking a highly skilled and dedicated Home Health Physical Therapy Assistant to join our team. As a Home Health Physical Therapy Assistant, you will play a vital role in providing high-quality patient care and support to individuals in their homes.
Responsibilities:- Under the supervision of a Physical Therapist, you will perform treatment and provide exercise instruction and patient education aimed at improving/enhancing the patient's well-being.
- You will instruct and aid patients in evidence-based treatment within the scope of the physical therapy assistant and according to state laws governing physical therapy assistants.
- Observes, records, and reports to the supervising PT, the nurse supervisor, and/or the physician the patient's response to treatment and changes in the patient's condition.
- Coordinates care with the other members of the healthcare team as appropriate.
- Instructs patient, family, caregiver, and other members of the health care team in the areas of therapy within the scope of the physical therapy assistant.
- Current licensed, registered, or certified to assist in the practice of physical therapy in the state of practice.
- Current CPR certification is required.
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
