This role is responsible for assessing the psychosocial and psychiatric requirements of patients and their families through clinical evaluations, crisis management, supportive counseling, and collaboration with the healthcare team and community professionals.
Utilizing strong communication and negotiation skills, this professional aims to deliver the appropriate services in a cost-effective manner, ensuring alignment with the goals of the patient, provider, payer, and healthcare organization.
Reports to the Manager of Child & Family Support Services.Key Responsibilities
PRIMARY DUTIES
Conducts timely psychosocial and psychiatric assessments of patients and families, gathering social, psychological, cultural, and financial information to inform care planning.
Identifies, interprets, and documents patient issues and strengths using a scientific knowledge base and relevant psychological/social/cultural concepts; organizes and prioritizes care and interventions; and secures appropriate resources to meet individual and family objectives.
Develops a therapeutic plan based on comprehensive assessments. Provides advocacy, referral, and discharge planning to address patient and family needs.Facilitates the treatment process by connecting patients and family members to suitable levels of care, support groups, and additional resources, while also providing direct crisis intervention.
Coordinates and implements patient and family care by collaborating with a multidisciplinary team, engaging with payors, and partnering with patients and families; establishes a comprehensive plan to address identified issues and goals; documents and communicates effectively; and educates patients and families to achieve established objectives and ensure smooth transitions across care levels.
Evaluates patients' medical insurance and other resources, initiating appropriate utilization review processes in placement decisions to protect patients' financial resources and ensure proper hospital reimbursement.
Negotiates care levels and lengths of stay with third-party payors by providing clear and accurate information regarding care recommendations and the program's capacity to deliver adequate patient care.
Ensures that the intake evaluation and referral process for prospective patients and families is appropriate by collaborating with medical providers, community resources, public agencies, and private clinicians to facilitate continuity of care and maintain ongoing relationships.
Works effectively as a team member within and across clinical settings to provide integrated healthcare services through communication, cooperation, and collaboration. Evaluates progress towards achieving desired patient care outcomes and addresses the termination and transition processes with patients and families.
Engages in performance improvement initiatives, research, and cost-effective resource utilization, and coordinates activities such as meetings to enhance patient care.
Takes responsibility for personal development by identifying, communicating, and seeking resources to meet individual learning needs.May provide medical on-call services as part of an on-call team.
Demonstrates support for the mission, values, and goals of the organization through behaviors that align with Connecticut Children's Standards.
Exhibits knowledge of age-related differences and needs of patients across specific populations from neonate through adolescence and applies this knowledge in practice.
Values cultural diversity and individual differences within the workforce.
Performs other related duties as assigned.
SUPERVISORY DUTIES
May supervise students during their professional internships.
May provide supervision for LCSW licensure
Qualifications
EDUCATION and/or EXPERIENCE REQUIRED
Master's degree in Social Work required.
Experience in psychiatric assessment, triage, and intake preferred.
A minimum of 2 years of relevant experience serving children and families is required.
Basic knowledge of medical conditions and healthcare experiences of specific patient populations is preferred.
Position Specific Job Education and/or Experience
LICENSE and/or CERTIFICATION REQUIRED
LCSW required.
Ability to meet internal credentialing requirements for individual and group or family therapy is required. Specific programs may require additional certifications.
Nonviolent Crisis Intervention (CPI) training certification is highly recommended within one year of hire and must be maintained thereafter.
