Catholic Health Service is seeking a skilled Home Infusion Pharmacist to join our team. As a Home Infusion Pharmacist, you will be responsible for providing pharmaceutical services to patients receiving infusion, specialty, or retail services in a manner that maximizes quality and patient safety.
Key Responsibilities:- Evaluate physician medication orders for appropriateness, potential interactions, and stability.
- Maintain patient databases with current and past medical history, clinical and laboratory data, and concurrent medication.
- Collaborate with the healthcare team to provide clinical and technical support.
- Document all communication with physicians, nurses, and patients in the medical record.
- Implement policies and procedures for the safe procurement, storage, distribution, and use of drugs and biologicals.
- Provide one-on-one drug therapy consultation with patients to identify, prevent, and resolve drug therapy problems.
- BS Pharmacy or Pharm D degree from an ACPE-accredited School of Pharmacy.
- Current licensure in the state of NY.
- One-year experience in home infusion, hospital, or managed care setting preferred.
- IV clinical experience preferred.
- Skill in use of electronic data management and computer systems.
- Strong clinical and analytical problem-solving skills.
- Outstanding telephone skills and etiquette.
Catholic Health Service offers a competitive salary range of $72.00 per hour, as well as a comprehensive benefits package, generous tuition assistance, and a defined benefit pension plan.
