Indiana University Health is seeking a skilled Nurse Case Manager to join our team at Arnett Hospital in Lafayette, IN. This inpatient position will work a mix of weekdays and alternating weekends, with 12-hour weekday shifts and 10-hour weekend shifts. The successful candidate will be responsible for planning and coordinating patient care from pre-admission to post-discharge, working collaboratively with the multidisciplinary team to ensure patients receive the appropriate level of care.
Key Responsibilities
- Accurately assess patient needs and develop effective care plans
- Coordinate with healthcare providers to ensure seamless transitions of care
- Communicate with patients and families to educate and empower them in their care
- Work with third-party payers to secure coverage and resolve any issues
- Administer education and prevention programs to promote healthy outcomes
Requirements
- Associate's Degree in Nursing required; BSN within 5 years of hire date for new ASN RNs
- Active Registered Nurse (RN) license in the state of Indiana or NLC RN license
- Certification in Case Management preferred
- Knowledge of InterQual Acute Level of Care Criteria and Federal guidelines
- 3-5 years of experience in acute care required
