About the Role

This position involves working as a Clinical Care Coordinator, where you will oversee all aspects of patients' home care and collaborate with patient care teams to develop and implement care plans.

You will need to possess excellent communication and interpersonal skills, as well as strong problem-solving and analytical abilities.

In addition to these skills, you should have at least one year of clinical experience as a registered nurse (RN), current RN license, specific to the state(s) you are assigned to work, minimum six months home health experience, current CPR certification, valid driver's license, reliable transportation and liability insurance.

**Responsibilities:

  • Coordinate and oversee all aspects of patients' home care
  • Collaborate with patient care teams to develop and implement care plans
  • Educate families on patient condition, needs and care
  • Perform head to toe physical assessments
  • Check vital signs and manage medications
  • Give injections, IV therapy or tube feedings
  • Administer and maintain catheters
  • Complete documentation timely and accurately
  • Supervise LPNs and HHAs
