Job Summary

We are seeking a skilled Nurse Practitioner or Physician Assistant to join our team as a Home Risk Assessment Specialist. In this role, you will conduct comprehensive health assessments of patients in their homes, identifying potential health risks and developing personalized care plans to improve their overall health and well-being.

Responsibilities
  • Conduct home visits to assess patients' health and identify potential risks
  • Develop and implement personalized care plans to address identified health risks
  • Collaborate with healthcare providers and other stakeholders to ensure seamless care coordination
  • Provide education and support to patients and their families to promote healthy behaviors and self-management
Requirements
  • Masters degree in Nursing or Physician Assistant studies
  • Current licensure as a Nurse Practitioner or Physician Assistant in the state of practice
  • Board certification in a relevant specialty
  • Minimum of 1 year experience as a Nurse Practitioner or Physician Assistant
  • Strong communication and interpersonal skills
