Job Summary:We are seeking a highly qualified Clinical Faculty member to join our team at the University of Illinois Hospital and Health Sciences System. The successful candidate will be responsible for providing pharmaceutical care to patients in the area of diabetes management, working in a telemedicine diabetes clinic, and participating in academic and research activities.
Key Responsibilities:- Provide pharmaceutical care to patients with diabetes in a telemedicine setting
- Work collaboratively with physicians and advanced practice nurses to provide comprehensive care
- Develop and implement evidence-based care plans in collaboration with the treating credentialed practitioner
- Initiate, modify, or discontinue drug therapy for treatment of diabetes to achieve therapeutic goals
- Order laboratory tests to assist in the management of diabetes
- Participate in academic and research activities, including didactic lectures and precepting entry-level Doctor of Pharmacy students
- Graduate of an ACPE-accredited College of Pharmacy PharmD program
- Completed a clinical residency and/or fellowship or have 2-3 years equivalent experience for practice in a clinical setting and/or education/experiential education
- Licensed, or eligible for licensure as a professional pharmacist in Illinois as of start date
- Knowledge and skills to qualify for a non-tenured clinical faculty position with the Department of Pharmacy Practice at the UIC College of Pharmacy
- PGY2 Ambulatory Care residency or 2-3 years of equivalent clinical experience
- Board certification
