Our Adult Home Health team provides comprehensive skilled nursing services to clients in their homes. We pride ourselves on our robust mentoring program for new hires and ongoing educational opportunities to enhance your professional growth.
Key Responsibilities:
- Deliver basic nursing care in home settings under the guidance of a Registered Nurse.
- Follow established nursing care plans to ensure optimal patient outcomes.
- Collaborate with a multidisciplinary team, including RNs, Physical Therapists, Social Workers, and administrative staff.
Travel Requirements: While the primary service area is assigned, occasional travel outside of this territory may be necessary. All mileage will be reimbursed at the federal gas rate.
Qualifications:
- Completion of an accredited LPN program.
- A minimum of two years of relevant experience in home health, public health, or hospice care.
- Current LPN licensure or compact licensure recognized by the State of Vermont.
- Previous experience in home care is preferred.
Essential Physical Requirements:
- Ability to walk, sit, stand, bend, lift, and move intermittently during working hours.
- Capability to push, pull, and lift up to 50 pounds.
- Comfortable navigating stairs of varying heights.
- Maintain balance and perform various physical tasks as required.
- Possess manual dexterity for equipment use and patient care.
- Visual and auditory acuity to assist individuals effectively.
