About the Role:

  • Develop long-term relationships with patients and their families, providing personalized care and support.
  • Collaborate with the healthcare team to ensure comprehensive patient care, leveraging innovative technology to monitor and manage complex medical conditions.
  • Perform assessments, administer medications, and educate patients and families on effective care strategies.

Requirements:

  • A current LPN license in Vermont.
  • Two or more years of directly related home/public health, hospice, and/or maternal/child health nursing experience.
  • Previous home care experience is a plus, but not required.
