We are seeking a skilled Advanced Practice Provider to join our dynamic team in the Chronic Kidney Disease (CKD) Program at Mass General Brigham. This is an exciting opportunity to make a meaningful impact on patients' lives while working in a collaborative and patient-centric environment.

About the Role

This full-time position, 40 hours per week, involves providing high-quality healthcare services to patients with CKD and End Stage Renal Disease (ESRD). The successful candidate will be responsible for clinical assessment, evidence-based kidney disease management, and coordination of care to ensure seamless transitions between CKD and ESRD. Additionally, the APP will contribute to the development, implementation, and operation of the CKD Program, providing clinical insight and oversight of day-to-day functions.

Key Responsibilities
  • Clinical assessment and management of patients with CKD and ESRD
  • Coordination of care to prevent unnecessary hospitalizations
  • Development and implementation of the CKD Program, including protocols, procedures, and policies
  • Providing comprehensive patient education and support
  • Collaboration with the Nephrology clinical team to ensure alignment with practice standards
Requirements

To be considered for this role, applicants must possess a Master's degree in nursing or physician assistant studies and be certified as a Massachusetts Nurse Practitioner or Physician Assistant. A minimum of 3 years of experience in clinical nephrology is preferred.
