The Vocational Nursing Specialist (LVN) plays a crucial role in identifying patient needs, upholding quality standards, and delivering essential patient care.
This position collaborates with Registered Nurses (RNs) to execute skills, procedures, and tasks that extend beyond their clinical scope.
Key Accountabilities:1.
Conduct basic assessments (data collection), engage in planning, implement interventions aligned with the care or treatment plan, and contribute to the evaluation of tailored interventions.
2.Identify, communicate, and document changes in patients' conditions, whether emergent or in response to care.
3.Gather and relay patient information during shift changes and in interactions with physicians, reporting significant incidents involving nursing staff, patients, and others.
4.Assist in developing and executing a teaching plan focused on self-care for patients.
5.Collaborate effectively with other healthcare team members to enhance patient and family well-being and support their choices.
6.Administer medications in accordance with organizational policies, adhering to the 5 Rights of Safe Medication Practice, and monitor for adverse drug reactions, reporting them to Pharmacy and the RN.
7.Ensure the completeness and accuracy of patient records.
8.Perform additional job-related duties as assigned.
Qualifications:
Education:
- High School Diploma, High School Equivalency (HSE), or completion of a CHS Approved Individualized Education Plan (IEP) Certificate is required.
- Completion of an accredited Vocational Nursing School program or licensure by equivalency is required.
- Current Basic Life Support (BLS) certification for Healthcare Providers by the American Heart Association (AHA) is required.
- Current State of California Licensed Vocational Nurse License is required.
- IV Therapy Certification (LVN) is required within 180 days of hire or transfer into the position.
- Advanced Cardiac Life Support (ACLS) certification is preferred.
- Basic Arrhythmia Course completion is required within 180 days of hire or transfer into the position for specific departments.
