Position Available: Care Coordinator - Amsterdam

We are in search of a dedicated professional to become part of our Care Coordination Services Agency in Amsterdam. As a Care Coordinator, you will be instrumental in evaluating client needs, developing care plans, and facilitating access to healthcare and social support services. Your key responsibilities will encompass:

  • Evaluating and creating care strategies for clients, addressing healthcare disparities
  • Coordinating services for individuals with complex chronic conditions
  • Connecting clients to vital resources and services
  • Assisting diverse populations, including those facing mental health challenges, homelessness, HIV/AIDS, Sickle Cell Anemia, or Substance Use Disorders
  • Thriving in a fast-paced environment while ensuring meticulous documentation

Position Details:

Compensation: $21.02 per hour
Work Schedule: Monday-Friday, 8:30 am - 4:30 pm

Qualifications:

  • Preferred Bachelor's Degree in Health, Education, or Human Services
  • 1-3 years of experience in case management
  • Proven ability to engage with diverse demographic groups
