As a Private Duty Nurse with our team, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.
Key Responsibilities:
- Provide one-on-one nursing care to patients in their homes, focusing on their unique needs and goals.
- Develop and implement individualized care plans to ensure the highest level of quality care.
- Collaborate with patients, families, and healthcare teams to ensure seamless care coordination.
- Monitor and report patient progress, identifying areas for improvement and implementing changes as needed.
- Maintain accurate and detailed records of patient care, including progress notes and medication administration.
Requirements:
