Position Summary
We are seeking a dedicated PERSONAL CARE ASSISTANT to become a part of our compassionate team. In this role, you will provide individualized support to individuals with disabilities, adhering to a personalized care plan within our group home environment. Your role is essential in delivering not just attentive care, but also in fostering meaningful connections with those you assist. You should possess strong organizational skills, attention to detail, and a commitment to creating a nurturing atmosphere that addresses the needs of our members. Excellent verbal and written communication skills are crucial for success in this position. Experience in group home settings and with behavioral health clients is advantageous.

Key Responsibilities
  • Support clients with daily living activities, including community outings, personal hygiene, and transportation
  • Assist in maintaining a clean and safe living environment by managing laundry, changing bedding, and providing personal care
  • Document and monitor clients' care, health status, and any incidents, ensuring clear communication with supervisors
  • Engage with clients through conversation and activities to promote mental well-being
  • Administer medications and provide transportation to medical appointments as necessary
  • Encourage participation in physical activities and exercises
  • Review and understand each client's individualized care plan to tailor your support effectively.

Required Qualifications
  • Completion of an accredited Home Health Aide program
  • High School Diploma or equivalent
  • Minimum of one year of relevant professional experience
  • Valid driver's license
  • Current CPR certification
