Job Summary

The Clinical Care Coordinator LPN will assist the CMHC and/or CPRC with the implementation and integration of Healthcare Home and its computer-based disease management programs in daily clinical operations.

Key Responsibilities
  • Assist HCH Director to develop and enhance wellness initiatives, health education groups, and activities for consumers, staff, and community.
  • Monitor critical chronic disease indicators by utilizing Population Health reports in Care Manager for assigned caseload.
  • Identify high-risk consumers through annual health screening and assessment and make referrals as needed.
  • Complete and track all required assessments and screenings for each consumer on caseload.
  • Maintain an accurate and up-to-date consumer record as outlined in the agency and site Policies and Procedure Manual.
  • Adhere to all applicable evidence-based practice models.
  • Orally communicate information effectively and accurately.
  • Ensure program operations align with the agency's certification (CARF, DMH) standards and strategic plan related to clinical care.
  • Abide by program and agency policies and procedures.
Requirements
  • Possession of a High School Diploma or equivalent.
  • Licensed Practical Nurse with current Missouri license.
  • Five years primary care experience in Nursing and experience with healthcare technology preferred.
  • Meet the minimum requirements to be clinically privileged by FCC Behavioral Health.
