Job Description
Role Summary:
We seek an experienced Registered Nurse to join our interdisciplinary team as a Professional Nurse – Patient Care Specialist. This vital role involves providing high-quality nursing care to patients in a variety of settings, collaborating with other healthcare professionals to ensure seamless transitions and optimal outcomes. As a key member of our team, you will be responsible for assessing patient needs, developing and implementing personalized care plans, and contributing to the delivery of evidence-based practice.
Responsibilities:
- Assessment and Planning: Conduct thorough assessments of patients' physical, emotional, and social needs, identifying areas for improvement and developing targeted interventions to address these gaps.
- Care Coordination: Collaborate with multidisciplinary teams to develop and implement patient care plans, ensuring timely and effective communication across all care settings.
- Patient Education: Provide empathetic and informative guidance to patients and their families, empowering them to take an active role in their care and promoting self-management skills.
- Documentation and Quality Improvement: Maintain accurate and comprehensive records, utilizing data to inform quality improvement initiatives and drive continuous learning.
- Leadership and Teamwork: Demonstrate leadership skills by mentoring new employees, participating in performance evaluations, and actively contributing to the development of unit policies and procedures.
Required Qualifications:
To be considered for this exciting opportunity, applicants must possess a current New York State license or permit as a Registered Professional Nurse, along with excellent interpersonal and analytical skills, and the ability to communicate effectively in both verbal and written formats. We also require experience in ACLS (ICU, Float pool) certification and BLS certification with de-escalation training.
