Registered Nurse RN, Guardian Home Health and Medical Services

We are seeking a highly skilled and compassionate Registered Nurse (RN) to join our team at Amedisys, a leading provider of home health, hospice, and high-acuity care. As a member of our care team, you will have the opportunity to make a meaningful impact on the lives of patients and their families, providing much-needed care in the comfort of their own homes.
Responsibilities- Coordinates and oversees all aspects of patients' home care, ensuring seamless delivery of services.
- Collaborates with patient care teams to develop and implement comprehensive care plans, tailored to meet the unique needs of each patient.
- Educates families on patient condition, needs, and care, empowering them to take an active role in their loved one's recovery.
- Performs head-to-toe physical assessments, checks vital signs, and manages medications, ensuring accurate and timely documentation.
- Administers and maintains catheters, gives injections, IV therapy, or tube feedings, and completes other medical procedures as needed.
- Supervises Licensed Practical Nurses (LPNs) and Home Health Aides (HHAs), providing guidance and support to ensure high-quality patient care.
- Regularly communicates patient progress to the clinical manager, physician, and care team, ensuring seamless coordination of care.
- One year of clinical experience as a Registered Nurse (RN), with a strong background in home health care.
- Current RN license, specific to the state(s) you are assigned to work.
- Minimum six months home health experience, with a proven track record of delivering high-quality patient care.
- Current CPR certification, valid driver's license, reliable transportation, and liability insurance.
- Able to assess patient status and identify requirements relative to age-specific needs, with a focus on delivering patient-centered care.
