At River Pointe of Trinity Healthcare and Rehabilitation, we are seeking a dedicated Rehabilitation Therapist to join our exceptional in-house therapy team. Your role will be pivotal in delivering comprehensive therapy services to our esteemed residents.

Key Responsibilities:

  • Conduct patient evaluations within forty-eight (48) hours following physician referral.
  • Formulate effective treatment strategies and secure approval for services from the referring physician.
  • Administer treatments in accordance with the physician's prescribed plan.
  • Oversee the work of Occupational Therapy Assistants and Aides in direct patient care and related activities, ensuring adherence to facility supervision policies.
  • Engage in Patient Care Conferences, Utilization Review Meetings, and Rehabilitation Conferences.
  • Document evaluations, daily treatment notes as required, weekly progress reports, and HFCA therapy certification/POC forms in line with facility protocols.
  • Record treatment charges daily as per facility guidelines.
  • Educate patients' families or nursing staff on maintenance programs.
  • Contribute to discharge planning.
  • Adhere to the Occupational Therapy Practice Act and facility policies and procedures.
Qualifications and Experience:
  • Must be a Licensed Occupational Therapist.
