As a skilled Home Health RN Case Manager, you will play a vital role in delivering high-quality, patient-centered care to individuals in the comfort of their own homes. Your expertise will enable patients to recover, rehabilitate, and regain their independence, empowering them to live healthier and happier lives.

Key Responsibilities:
  • Provide comprehensive admission, case management, and follow-up skilled nursing visits to home health patients, ensuring seamless care transitions and optimal outcomes.
  • Collaborate with interdisciplinary teams, including physicians, therapists, and home health aides, to develop and implement personalized care plans that address patients' unique needs and goals.
  • Conduct thorough assessments, identify areas for improvement, and implement evidence-based interventions to enhance patient outcomes and satisfaction.
  • Communicate effectively with patients, families, and healthcare providers to ensure timely and accurate information exchange, promoting a culture of transparency and trust.
  • Manage and evaluate patient care plans, revising them as needed to ensure alignment with changing patient needs and goals.
  • Coordinate and oversee the work of home health aides, certified home health aides, and personal care workers, providing guidance and support to ensure high-quality care delivery.
  • Document patient care and progress accurately and thoroughly, maintaining up-to-date records and reports to inform care decisions and quality improvement initiatives.
Requirements:
  • Diploma, Associate, or Bachelor's Degree in Nursing
  • A minimum of one year of nursing experience, preferably in med-surg, ICU, ER, or acute care settings
  • Strong organizational and communication skills, with the ability to work effectively in a fast-paced, dynamic environment
  • Current and unrestricted Registered Nurse licensure, as well as CPR certification
  • A valid driver's license, auto insurance, and reliable transportation
