Job Summary:

We are seeking a highly skilled and compassionate Part Time RN Case Manager to provide quality and comprehensive care to our clients in the comfort of their own homes.

As a Part Time RN Case Manager, you will work closely with our team to ensure that our clients receive the highest level of care and support.

Responsibilities:
  • Conduct initial assessments for new clients and develop a comprehensive care plan
  • Provide skilled nursing care, including medication management and wound care, according to physician orders
  • Coordinate and collaborate with other healthcare professionals, such as therapists and social workers, to ensure the best possible care for our clients
  • Monitor and evaluate client progress and adjust care plans as needed
  • Educate clients and their families on proper care and self-management techniques
  • Maintain accurate and timely documentation of all care provided
  • Communicate effectively with the BrightStar Care team to ensure client needs are met
  • Adhere to all policies, procedures, and standards of nursing practice
Requirements:
  • Current and unrestricted RN license in the state of Indiana
  • Minimum of 1 year of nursing experience, preferably in home health or hospice care
  • Strong clinical assessment and critical thinking skills
  • Personal Care Service Experience
  • Knowledge of Indiana Medicaid a plus
  • Ability to work independently and manage time effectively
  • Excellent communication and interpersonal skills
  • Proficient computer skills for documentation purposes
  • Valid driver's license, reliable transportation, and proof of car insurance
