Job Summary

We are seeking a skilled Licensed Vocational Nurse (LVN) to join our team as a Home Health Professional. As an LVN Home Health Care Specialist, you will conduct assessments, administer medications, monitor patients' health, educate on health management, maintain records, offer support, and work collaboratively under RN supervision.

Key Responsibilities
  • Conduct thorough assessments of patients' health needs and develop personalized care plans
  • Administer medications and treatments as prescribed by physicians
  • Monitor patients' health and report any changes or concerns to RN supervisors
  • Provide education and support to patients and their families on health management and self-care
  • Maintain accurate and detailed records of patient interactions and care provided
  • Collaborate with RN supervisors and other healthcare professionals to ensure seamless care delivery

Requirements
  • Current and unrestricted CA LVN License
  • 1 year of LVN experience in a direct patient care setting
  • Current AHA CPR certification
  • Valid Driver's License
