Job Summary

We are seeking a highly skilled Registered Nurse Case Manager to join our team at Medical City Arlington. As a key member of our healthcare team, you will be responsible for coordinating patient care, managing length of stay, and ensuring appropriate resource utilization.

Key Responsibilities
  • Conduct comprehensive assessments of patient needs, including psychosocial, medical, and discharge requirements.
  • Develop and implement individualized care plans, collaborating with the multidisciplinary team to ensure seamless transitions and optimal outcomes.
  • Coordinate activities to promote quality outcomes, patient throughput, and resource utilization, while maintaining a balance of optimal care and resource management.
  • Collaborate with Social Services to ensure post-acute medical needs and level of care are appropriate.
  • Make timely referrals to Social Services when risk factors for psychosocial determinants of health are identified.
  • Involve patients, families, and responsible individuals in identifying and clarifying needs and expectations to develop mutual and realistic goals.
  • Evaluate progression of care using evidence-based tools and approved criteria, escalating issues through the established chain of command.
  • Make appropriate referrals to third-party payer, disease, and case management programs for recurring patients and those with chronic disease states.
  • Facilitate patient throughput with an ongoing focus on effective care transitions, quality, and efficiency.
  • Document professional recommendations, discharge plans, care coordination interventions, and case management activities to effectively communicate with the healthcare team.
  • Align patient needs with available resources to ensure safe discharge/transition.
  • Act as a liaison through effective and professional communications between and with physicians, patients/families, hospital staff, and outside agencies.
  • Direct activities to identify and provide for the needs of the under-resourced patient population, including patient education activities, patient assistance programs, and community-based resources.
Requirements
  • Registered Nurse with current TX state license required.
  • Graduate of an accredited nursing school, Associate's Degree required, Bachelors degree preferred.
  • 3+ years of clinical hospital nursing experience required.
  • 2 years of Case Management experience required.
  • 2 years of critical care experience preferred.
  • Certification in case management preferred.
  • InterQual experience preferred.
