Key Responsibilities
  • Conduct initial and ongoing discharge screens/evaluations to determine patient and family needs.
  • Collaborate with the healthcare team to develop and implement discharge plans.
  • Assess patient emotional status, social support systems, and identify areas for improvement.
  • Prioritize barriers to discharge and coordinate resources accordingly.
  • Communicate with patients, families, and caregivers throughout the admission to discharge process.
  • Perform clinical reviews and maintain documentation.
  • Participate in performance improvement processes.
Requirements
  • Associate's degree in Nursing required; Bachelor's degree preferred.
  • Current/active North Carolina or compact RN license.
  • Ability to understand, communicate, and react effectively to patient and family needs.
  • Critical thinking and ability to work independently.
  • Collaborative mindset and ability to work with internal and external resources.
