RN-Case Management Specialist

Wilson Medical Center is seeking a skilled RN-Case Management professional to join our interdisciplinary healthcare team. As a Case Manager RN, you will play a vital role in facilitating the delivery of healthcare services, integrating utilization management, case management, and discharge planning to promote optimal clinical and financial outcomes.
Key Responsibilities- Conduct initial and ongoing discharge screens/evaluations to determine patient and family needs.
- Collaborate with the healthcare team to develop and implement discharge plans.
- Assess emotional status, quality of relationships, and social support systems to identify areas for improvement.
- Prioritize immediate plans for barriers to discharge, including medication availability, shelter, and primary care physician coordination.
- Communicate with patients/families from admission to discharge.
- Perform clinical reviews based on the Utilization Review Plan, with follow-up actions identified for medical necessity and level of care.
- Maintain clinical and financial documentation.
- Participate in performance improvement processes and other duties as required.
- Associate's degree (required), Bachelor's degree (preferred).
- Current/active North Carolina or compact RN license.
- BLS (optional).
