King's Daughters is seeking a skilled Social Worker to join our team. As a Social Worker, you will play a vital role in assisting patients and their families with issues related to illness or recovery. Your expertise will help us understand social, emotional, and environmental factors that impact patient care.

This is a dynamic role that requires strong communication skills, empathy, and a passion for delivering exceptional patient care. You will work closely with our interdisciplinary treatment team to provide education, advocacy, and support to patients and their families.

Key responsibilities of this role include:

  • Completing individualized psychosocial and discharge planning assessments to identify emotional, social, and environmental factors related to diagnosis, illness, treatment, and life situation.
  • Providing interventions as indicated in the completion of psychosocial and discharge planning assessments, including but not limited to crisis intervention, education, advocacy, referral management, and/or mediation.
  • Assisting with screening, identification, and intervention in cases involving suspected abuse, neglect, intimate partner violence, exploitation, and sexual assault.
  • Collaborating with and providing education to the interdisciplinary treatment team regarding patients' assessed needs, care transition options, and patient choice.
  • Maintaining accurate and up-to-date records of patient progress, including measurable goals and interventions.

To be successful in this role, you will need:

  • A Bachelor's degree in Social Work and a current license as a Licensed Social Worker (LSW) in the state of Kentucky.
  • At least 2 years of related experience or training in a healthcare setting, preferably in a social work role.
