Advanced Heart Failure Care Specialist

About the Role: We are seeking an experienced Cardiology Nurse Practitioner to join our outpatient Heart Failure Clinic in Tyler, TX. As a key member of our team, you will work collaboratively with healthcare professionals to provide exceptional patient care and contribute to the growth and development of our clinic.
Key Responsibilities:
- Provide high-quality patient care, including assessments, diagnoses, and treatment plans, under the supervision of a physician.
- Collaborate with cardiologists and other healthcare professionals to develop and implement individualized care plans for patients with heart failure.
- Educate patients and their families on disease management, medication adherence, and lifestyle modifications to improve cardiovascular health.
- Participate in multidisciplinary teams to enhance patient outcomes and quality of life.
Requirements:
- Certification as a Family Nurse Practitioner (FNP), Acute Care Nurse Practitioner (ACNP), or Physician Assistant (PA) is required.
- A minimum of 2 years' experience in cardiology, preferably in a similar setting, is preferred.
- Familiarity with Epic EMR system is essential.
Compensation Package:
- A competitive salary range of $120,000 - $150,000 per year, based on experience.
- A quality bonus of 5% ($6,000 - $7,500) paid twice a year.
