Job Summary

We are seeking a dedicated and compassionate Pediatric Home Health Nurse to join our team. As a Licensed Practical Nurse (LPN) or Registered Nurse (RN), you will provide direct, one-on-one medical care to our patients and their families in the comfort of their own homes.

Responsibilities
  • Focus on the medical needs and treatment of infants, children, and/or adolescents in their home, according to the active plan of care.
  • Conduct ongoing patient care and assessments to ensure the best possible outcomes.
  • Administer prescribed medication, treatments, and therapies as needed.
  • Coordinate care with other healthcare professionals to ensure seamless service delivery.
  • Educate family members on patient clinical care to enhance positive outcomes.
  • Preventative initiatives to protect quality of care for patients.
