Job Summary

We are seeking a skilled Pediatric Private Duty LPN to provide high-quality patient care in the comfort of their own homes. As a member of our team, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

Responsibilities
  • Provide direct nursing care to patients in their homes, following the plan of care and in coordination with other members of the healthcare team.
  • Document patient visits thoroughly and completely, ensuring accurate and timely reporting of patient needs and changes in condition.
  • Participate in the agency's care coordination process, communicating effectively with patients, families, physicians, and other clinicians to ensure seamless care delivery.
  • Ensure confidentiality of patient health information and maintain a professional demeanor at all times.
Requirements
  • Current LPN licensure in the state of Tennessee.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
  • Current CPR certification.
  • Previous home care experience preferred.
