Job Summary:

This position is eligible for a $15,000 sign-on bonus.

We are seeking a highly skilled and compassionate Social Work Case Manager to join our team at Kaiser Permanente in Timonium, MD. As a key member of our healthcare team, you will play a vital role in coordinating care for our members, ensuring they receive the best possible outcomes.

Essential Responsibilities:
  • Collaborate with healthcare professionals to identify and address the social determinants of health that impact our members' well-being.
  • Conduct comprehensive psychosocial assessments to evaluate patients' goals, social support systems, resources, health status, functional limitations, psychological status, environmental factors, and response to treatment.
  • Develop and implement individualized plans of care that promote optimal functioning and enhance quality of life.
  • Facilitate access to community resources and services to support our members' needs.
  • Ensure timely and accurate documentation of case management activities in compliance with organizational standards.
  • Analyze patient and program outcomes to identify opportunities for improvement.
  • Provide education and support to patients, families, and caregivers on end-of-life issues, Advanced Directives, and other relevant topics.
  • Maintain current knowledge of laws, regulations, and policies related to social work practice in the local market.
Requirements:
  • Master's degree in social work (MSW) required.
  • Minimum one year of clinical social work experience in a healthcare setting required.
  • Licensed Independent Social Worker (District of Columbia) within 6 months of hire OR Licensed Graduate Social Worker (District of Columbia) within 6 months of hire.
  • Licensed Master Social Worker (Maryland) within 6 months of hire.
  • Licensed Master's Social Worker (Virginia) within 6 months of hire.
Preferred Qualifications:
  • Experience with computer software programs in a Windows environment preferred.
  • Knowledge of community systems and resources in the defined service area preferred.
  • Knowledge of regulatory issues for the Mid-Atlantic area preferred.
