Job DescriptionWe are seeking a skilled and compassionate Clinical Social Worker to join our team. In this role, you will:
- Provide direct and indirect services to patients, families, and community members, ensuring their needs are met and they receive the support they require.
- Assist in program planning and development for specific service populations, helping to drive positive change and outcomes.
- Coordinate discharge planning and family evaluations, working closely with healthcare professionals to ensure seamless transitions.
- Conduct behavioral health assessments and chart reviews, using your expertise to inform care decisions.
- Engage in social service activities, offering support and guidance to service populations as needed.
To be successful in this role, you will need:
- A Master's Degree in Social Work from an accredited school.
- Basic knowledge of keyboarding and computer functions.
- Minimal knowledge in software, hardware, or information systems.
- Good communication skills, with the ability to work effectively with diverse stakeholders.
- Positive customer relations, with a focus on delivering exceptional care and support.
- Critical analysis, with the ability to evaluate complex situations and develop effective solutions.
- Effective organizational skills, with the ability to prioritize tasks and manage multiple projects.
- Comfortable working independently, with a strong sense of self-motivation and accountability.
- Ability to effect change, with a passion for driving positive outcomes and making a difference in the lives of others.
- Certifications such as CPR - AHA Healthcare Provider, Certified Clinical Social Worker (CCSW), Graduate Social Worker (LGSW), and CJIS fingerprint clearance.
