- Conduct comprehensive assessments to determine patients' physical and psychosocial needs
- Develop and implement individualized care plans to promote patient independence and well-being
- Collaborate with patients, families, and healthcare teams to ensure seamless care coordination
- Stay up-to-date with the latest clinical guidelines and best practices in home health care
- Assess patients' health status and develop care plans to address their needs
- Provide education and support to patients and families to promote healthy behaviors and self-care
- Document patient information accurately and timely to ensure continuity of care
- Participate in quality improvement initiatives to enhance patient outcomes and satisfaction
- Develop and maintain knowledge of third-party payer guidelines and regulations
- Minimum of one year of acute nursing experience
- Home care nursing experience preferred
