Job Description:
As a Homeless Outreach Specialist, you will play a critical role in identifying individuals who are experiencing homelessness or at risk of becoming homeless, and providing them with support services to help them access treatment and achieve stability. You will work closely with community service agencies, shelters, and other stakeholders to ensure seamless coordination of care and resources.

Key Responsibilities:
• Provide short-term counseling and case management services to individuals experiencing homelessness or at risk of becoming homeless.
• Develop and maintain relationships with community service agencies, shelters, and other stakeholders to ensure effective coordination of care and resources.
• Conduct outreach to identify individuals in need of support services and connect them with available resources.
• Collaborate with interdisciplinary teams to develop and implement individualized plans of care.
• Maintain accurate records and reports to track progress and outcomes.

Requirements:
• Bachelor's degree in psychology, social work, or a related field.
• Minimum of 2 years of experience working with individuals receiving mental health services.
• Valid driver's license and automobile with liability insurance.
• Excellent communication and interpersonal skills.
