The Registered Nurse (RN) serves as an integral member of the primary care team, reporting directly to the Clinical Services Manager (CSM). This position supports the Advanced Practice Clinician (APC) and our members in coordinating care services. The RN collaborates closely with all team members and may assist multiple providers. Under the guidance of the provider, the RN undertakes activities delegated by the provider or the team CSM. The RN is knowledgeable about and supports the Care Program Models including the Institutional Special Needs Plan (ISNP), Institutional Special Needs Plan (IESNP), and Transitions to Skilled (TTS), working collaboratively with the team to achieve program objectives. This role will provide support to our members both telephonically and in-person in various settings.

Key Responsibilities:

  • Establish and nurture communication and a trusting collaborative relationship with the Interdisciplinary Care Team (ICT), member/Responsible Party (RP), and primary caregiver staff in the Assisted Living Community (ALC) and APC.
  • Coordinate with various organizational units for seamless benefit coordination, including utilization review staff, provider relations liaison, and pharmacy.
  • Assess member status and assist in achieving progress towards goals; modify the Individual Care Plan (ICP) as needed.
  • Perform administrative tasks on behalf of the Primary Care Provider (PCP) and/or APC.
  • Engage in program effectiveness evaluation and strategic planning.
  • Conduct member visits and rounding as directed by the APC.
  • Communicate member status with the APC for evaluation and ICP updates.
  • Interact with members, families/RPs, primary caregivers, and other ICT members during each visit.
  • Assist with clinical and quality responsibilities for members, such as addressing gaps in care.
  • Attend meetings on behalf of the APC and relay updates to the ICP with the ICT.
  • Provide clinical educational programs for facility staff.
  • Convey information on behalf of the APC to members, families/RPs, and primary caregiver staff at the ALC.
  • Coordinate and collaborate with the health plan/client.
  • Assist with Advance Care Planning (ACP).
  • Collect data including functional status, BMI, vital signs, and other clinical information, ensuring results are entered into the Electronic Medical Record (EMR) for APC review.
  • Facilitate care coordination activities within the ALC to meet member needs, including external provider appointments, therapy requirements, durable medical equipment (DME), and transportation.
  • Stay informed about all Model of Care (MOC), documentation, and policy updates communicated via email, newsletters, and team meetings.
  • Maintain proficiency in utilizing clinical resources independently, including Resource1, iCoreCodeGenius, Sparq, Learnsource, and other internal Optum sites.
  • Ensure confidentiality of health information in accordance with state and federal regulations, including HIPAA.

Required Qualifications:

  • Current unrestricted Registered Nurse license in Washington state.
  • Minimum of 3 years of RN experience.
  • At least 1 year of case management experience.
  • Experience with electronic medical records.
  • Basic proficiency in Microsoft Office and ability to navigate in a Windows environment.
  • Demonstrated excellent communication and customer service skills.
  • Valid driver's license with the ability to travel locally 75% of the time.

Preferred Qualifications:

  • Bachelor of Science in Nursing (BSN).
  • Experience in post-acute care settings, such as long-term care or hospice, and/or case management experience as an RN.
  • Experience working with the geriatric population.
  • Familiarity with the long-term care business environment.

Washington Residents Only: The salary range for this position is $58,300 to $114,300 annually. Compensation is based on various factors including local labor markets, education, work experience, and certifications. UnitedHealth Group adheres to all applicable minimum wage laws.
